Care Practitioner Part-Time

Summary:

We are seeking a dedicated Care Practitioner to join our team in Woodbridge and surrounding areas. This permanent, full-time role is essential in providing high-quality care and support to our clients. The Care Practitioner will play a vital role in enhancing the well-being of individuals, ensuring they receive compassionate and professional assistance in their daily activities.

Responsibilities:

• Provide personal care and support to patients, including assistance with daily living activities.
• Monitor and record patients’ health and progress, reporting any changes to the healthcare team.
• Administer medications as prescribed and ensure compliance with care plans.
• Assist in the development and implementation of individual care plans.
• Maintain a clean and safe environment for patients, adhering to health and safety regulations.
• Communicate effectively with patients, families, and healthcare professionals to ensure continuity of care.
• Participate in training and development opportunities to enhance skills and knowledge.

Requirements:

• Previous experience in a Carer role, preferably in domiciliary care.
• Strong communication and interpersonal skills.
• Ability to work effectively as part of a team and independently.
• Compassionate and client-focused approach to care.
• Valid UK driving licence and access to a vehicle.

Hours & Pay:

• 2 week shift pattern including alternate weekends
• Shifts are a combination of 8am-2pm and 3pm-9pm
• 4 shifts per week
• Pay per shift (rather than per client visit)
• Mileage paid between client visits at a rate of 45p per mile
