This is primarily a community caseload management role, working directly with older adults, Holocaust survivors, people living with dementia, and individuals experiencing social isolation or declining wellbeing. You will hold and manage your own caseload, providing one-to-one support, assessments, personalised support planning, advocacy, and ongoing interventions that help people remain independent and connected to their communities.

Responsibilities:

You’ll provide person-centred, trauma-aware support, helping individuals stay connected, independent and valued. You will:

  • Build trusted relationships and reduce isolation
  • Co-create personalised support plans
  • Deliver community activities and wellbeing support
  • Provide culturally respectful, trauma-informed care

You’ll play a key role in supporting Holocaust survivors with sensitivity, focusing on trust, safety and dignity.

    Qualifications:

    • Experience in social care or a related field, particularly with older adults or people affected by trauma.
    • Empathetic, culturally aware and a strong communicator
    • Empathy and understanding of the challenges faced by older adults and Holocaust survivors.
    • Ability to work independently and as part of a team, demonstrating initiative and problem-solving skills.
    • Knowledge of local resources and services available to older adults and Holocaust survivors.
    • A relevant qualification in social care, health, or a related field is desirable but not essential.
