The Team Leader is responsible for the day-to-day leadership of Supported Independent Living (SIL) homes and Community Access Supports (CAS), ensuring participants receive high-quality, person-centred supports that align with NDIS Practice Standards, participant goals, Behaviour Support Plans, and Velocity Support Services’ policies. The Team Leader provides frontline leadership, mentors support workers, ensures compliance, coordinates service delivery, and acts as the first point of contact for staff and participants.

Summary:

We are seeking a dedicated Team Leader for Supported Independent Living (SIL) and Community Access Supports in Ipswich, Queensland. This part-time contract role is essential for overseeing the delivery of high-quality support services to individuals with disabilities, ensuring they achieve their personal goals and enhance their independence. The Team Leader will play a pivotal role in fostering a supportive environment and leading a team of disability support workers to provide exceptional care and assistance.

Responsibilities:

  • Lead and supervise a team of disability support workers, ensuring effective service delivery and adherence to organisational policies.
  • Develop and implement individual support plans tailored to the needs of clients, promoting their independence and community engagement.
  • Conduct regular assessments and reviews of client progress, adjusting support strategies as necessary.
  • Facilitate training and development opportunities for team members to enhance their skills and knowledge.
  • Maintain accurate documentation and reporting of client activities and team performance.
  • Collaborate with external agencies and stakeholders to ensure comprehensive support for clients.
  • Promote a positive and inclusive environment that respects the rights and dignity of all individuals.
  • SIL Rostering & Emergency shift coverage.
  • Family & Stakeholder Engagement
  • Compliance Responsibilities

Qualifications:

  • Cert IV / Diploma / EN or RN with leadership responsibilities
  • Previous experience as a team leader, coordinator, manager essential.
    • Previous experience as a Nurse, disability support worker or in a similar role within the social care sector.
    • Demonstrated leadership skills and the ability to motivate and manage a team effectively.
    • Strong understanding of the principles of Supported Independent Living and community access supports.
    • Excellent communication and interpersonal skills, with the ability to build rapport with clients and their families.
    • Relevant qualifications in social care, community services, or a related field are preferred.
    • Current First Aid and CPR certification.
    • A valid driver’s licence and access to a reliable vehicle.
