Job description: Support Worker – Gilgandra, New South Wales, Australia (Up to 30 hrs/week) Female Client | Daily Care Assistance

Seed Disability Services (NDIS Registered Provider) is seeking a reliable and compassionate Disability Support Worker to support a -year-old participant with community access and daily wellbeing supports.

Key Duties: Support attendance at GP and health appointments Assist with community outings, events, and appointments of the participant’s choosing Provide daily wellbeing check-ins (non-clinical) and report concerns appropriately Assist with light domestic tasks Deliver supports one-on-one and in small group settings Complete NDIS-compliant shift notes

Requirements: NDIS Worker Screening Check First Aid & CPR Driver’s licence & reliable vehicle (preferred) Person-centred, reliable, and professional
