Summary:

The Community Health Worker is responsible for providing culturally appropriate healthcare services and information to community members with the goal of improving health outcomes. They work in a variety of settings, including clinics, hospitals, community centers, and outreach programs.

Responsibilities:

• Conduct community assessments to identify healthcare needs and develop health education programs.
• Organize health screenings and outreach events in the community to provide access to health resources and services.
• Identify health disparities and work with local health organizations to improve access to care for disenfranchised groups.
• Build trusting relationships with community members, providing education on healthy living, chronic disease management, preventive care, and disease self-management.
• Advocate for individual and community healthcare needs to healthcare providers, policymakers and other relevant organizations.
• Assist with coordination of referrals to medical specialists, social services, and other community resources.
• Maintain accurate records of interventions, referrals and care coordination efforts.

Qualifications:

• High school diploma or equivalent, with some formal training in public health, community organizing or related field or a bachelor’s degree in public health, community health or a related field preferred.
• Bilingual capability in English and other languages, depending on the community served, is a plus.
• Ability to work independently and create efficient workflows to manage tasks and responsibilities.
• Experience interacting with diverse communities and building positive relationships.
• Proficient with computer programs and electronic health record systems.
• Understanding of HIPAA privacy rules and ability to maintain confidentiality.
