About the Role

Work alongside Elders, families and carers to navigate My Aged Care and connect with culturally appropriate aged care services.

In this vital role, you will:

  • Work alongside Elders, families and carers to navigate My Aged Care and connect with culturally appropriate aged care services.
  • Provide face-to-face, community-based support to help Elders access assessments, care planning, CHSP, HCP, respite and residential care.
  • Advocate for Elders and families to ensure cultural needs and family obligations are respected.
  • Deliver yarning groups and information sessions to raise awareness of aged care rights and entitlements.
  • Liaise with health services, hospitals, and community organisations to coordinate support.

Knowledge of My Aged Care pathways and eligibility criteria for CHSP, HCP, respite and residential care.

Essential

Strong ability to work effectively with Aboriginal and Torres Strait Islander Elders, families, and communities in a culturally safe and trauma-informed way.

Proficient record keeping and reporting skills, including data entry for client management systems.

  • Strong advocacy, communication and facilitation skills.
  • Experience in community services, aged care or health navigation roles.
  • Good record keeping and reporting skills.
  • Ability to work autonomously and as part of a team.
  • Current WA Driver’s Licence, National Police Clearance (within 3 months), Working With Children Check, and relevant vaccinations.

Desirable

  • Aboriginal and/or Torres Strait Islander applicants strongly encouraged.
  • Demonstrated experience in developing and delivering culturally appropriate information and yarning groups.
  • Understanding of relevant legislation and standards pertaining to aged care and Aboriginal and Torres Strait Islander health and wellbeing.
  • Community connections and lived experience supporting Elders.
  • Experience working in an ACCHO or Aboriginal community setting.
  • Qualifications in Community Services, Aged Care or similar.
