At Marymead CatholicCare, our CHOICES & Aged Care team is committed to empowering people to live fulfilling, independent lives. We support people with disability, older persons, and young people with trauma backgrounds through flexible, person-centred services tailored to individual goals and aspirations. We are seeking a Support Coordinator to join our team in a three month fixed term, full time role.

About the Role:

The CHOICES Support Coordinator will engage NDIS Participants and their families, who have support coordination in their plans, in the development of their CHOICES Service Agreement and support arrangements to ensure that their needs and wishes are addressed, and NDIS goals achieved. The Support Coordinator will promote individuality, positive outcomes, choice, and control.

Key Responsibilities:

• Facilitate initial assessments/reviews and develop a thorough service agreement and schedule of support to guide CHOICES support provision that will meet the participants individual needs and goals.
• Support participants to research and engage services to assist in meeting their plan goals that are in line with their NDIS Plan budget.
• Ensure all participant information, shift plans, support plans and case notes are up to date and accurately recorded.
• Liaise closely with the scheduling team to initiate and monitor services for individuals without Support Coordination in their plans.
• Be a contact point for participants, their nominee/family, support workers and other service providers.
• Review service agreements.

About You

You are a caring and resilient professional who is passionate about supporting people to achieve their goals. You thrive in a team environment while also being confident working independently.

You will also require:

• A Certificate in Disability/Mental Health/Community Services, or other relevant tertiary Qualification, or expereience in support coordination or other disability.NDIS services.
• Knowledge and understanding of the National Disability Insurance Scheme (NDIS).
• Hold a certificate demonstrating the successful completion of the NDIS Worker Orientation Module ‘Quality, Safety and You’.
