Description

About Golden Heart Services

Golden Heart Services is a registered NDIS provider delivering Supported Independent Living (SIL), 24/7 rostered care and community supports across NSW. We believe in supporting people with disability to live with independence, dignity and choice, and we’re looking for a hands-on leader to help our clients and team thrive.

About the role

As Client Services Coordinator, you’ll lead the day-to-day delivery of high-quality supports across our SIL homes and community services in Greater Sydney. Reporting to the Senior Manager, you’ll supervise a team of Support Workers, coordinate care and rosters, and make sure everything we do meets the NDIS Practice Standards.

This is a varied, people-focused role where you’ll work closely with clients, families, support coordinators and external providers, and step in on shift when needed to keep care running smoothly.

What you’ll be doing

  • Leading, supervising and supporting a team of Support Workers, including recruitment, training, rostering and performance management
  • Assessing client needs and developing individualised support plans that promote independence, community inclusion and quality of life
  • Coordinating and overseeing services including personal care, meal preparation, medication management and household tasks
  • Connecting clients with education, training, health and community resources
  • Acting as the main point of contact between clients, families, support coordinators and other providers
  • Ensuring compliance with NDIS Practice Standards, WHS legislation, emergency response, and infection control protocols
  • Keeping accurate records on client care, staff performance and household operations
  • Filling in on support shifts when needed to ensure continuity of care

What we’re looking for

  • Previous experience in a coordination or team leader role within the disability sector, with demonstrated success in Supported Independent Living (SIL) environments (highly regarded).
  • Strong leadership skills and the ability to motivate and empower a team
  • A genuinely client-centred approach, with demonstrated skills in effective problem-solving and conflict resolution within a care environment.
  • Excellent written and verbal communication
  • Solid understanding of the NDIS and the NDIS Quality and Safeguards Commission requirements
  • Confidence using Microsoft Office, client management software and digital record-keeping
  • Proficiency in using rostering software and client management systems relevant to the NDIS sector.

Essential requirements

  • NDIS Worker Screening Check
  • Working with Children Check (NSW)
  • First Aid and current CPR
  • Current driver’s licence [and access to a reliable vehicle, if required]
  • Completion of the NDIS Worker Orientation Module “Quality, Safety and You”
  • Infection control training (or willingness to complete)
  • Willingness to undergo a National Police Check, if not already covered by the NDIS Worker Screening Check.
  • Demonstrated ability to manage complex rosters and team scheduling in a 24/7 care environment.
  • Proficiency in utilising digital case management systems and maintaining accurate client records in line with NDIS guidelines.
