Support at Home Care Partner – Northern Sydney Region

For six years, we’ve supported older people across Northern Sydney. We’re now building on those foundations — strengthening local community connections and reaching more older people who need support.
This is a hybrid, locally based role, combining working from home with being out in the Northern Sydney community, with a particular focus on Hornsby, Ryde, Denistone, Macquarie Park, Marsfield, Eastwood, Epping. You’ll have the autonomy to manage your region and build relationships while remaining connected to and supported by the broader FOCUS Connect team.
About the Role
As our Care Partner – Northern Sydney, you’ll combine care partnering with community engagement and growth.
You’ll build relationships across the aged care, health and community sectors to strengthen referral pathways and connect more older people with the support they need. You’ll also work directly with clients and families to understand what matters to them, provide meaningful choice and help them make the best use of their eligible aged care funding.
Your responsibilities will include:
- Build strong community and referral networks across Northern Sydney driven by purpose to reach older people in need and connect them with the right services and supports.
- Identify opportunities to increase CHSP referrals, grow Support at Home client numbers and strengthen FOCUS Connects local presence.
- Manage a portfolio of Support at Home and CHSP clients, building trusted relationships with clients, families and representatives.
- Develop and review person-centred care plans that support client goals, choice and effective use of available funding.
- Work collaboratively with our support teams to deliver responsive, well-coordinated services while maintaining high standards of quality and compliance.
About You
We’re looking for someone who genuinely loves connecting with people and being part of their local community.
You’ll be self-motivated, organised and comfortable working independently, with the initiative to get out into the community, build relationships and create opportunities. You’ll be equally comfortable connecting with local organisations as you are sitting with an older person and their family to understand what matters to them.
You’ll bring:
- Experience in aged care, home care, community services, health or a related sector, with a track record of achieving positive outcomes.
- Experience in care coordination, case management or a similar client-facing role.
- Strong networking and relationship-building skills, with the confidence to establish new referral pathways.
- A results-focused, self-motivated approach and the ability to work independently and manage competing priorities.
- A genuine connection to Northern Sydney and an understanding of the local community.
- Sound knowledge of person-centred care and contemporary aged care practice.
- Knowledge of Support at Home, CHSP and the Aged Care Quality Standards will be highly regarded.
A relevant tertiary qualification in nursing, allied health or a related discipline is desirable but not essential.
You’ll also require relevant employment screening and checks, or be willing to obtain them, and hold a current driver’s licence with access to a comprehensively insured vehicle.
