🌟About the Role

We are seeking experienced and compassionate Support Workers (in line with participant choice and support requirements) to join our dedicated team, supporting individuals living with multiple complexities.

This role involves providing person-centred support to residents experiencing progressive physical and cognitive decline, as well as behaviours of concern. You will work within a small, supportive team to deliver high-quality daily living support in a safe, respectful, and nurturing environment.

This position will be offered on a full-time basis to the right candidates.

💼 Key Responsibilities

  • Provide personal care and daily living support with dignity and respect.
  • Support individuals displaying behaviours of concern in line with positive behaviour support plans.
  • Manual handling tasks such as bed positioning, personal care, hoist operation, commode assistance and manual/power wheelchair support.
  • Changing bed linen, washing, ironing, meal preparation and cooking fresh wholesome meals
  • Assist with meal-time management, including modified diets, texture assessments, and safe swallowing practices.
  • Administer and document medication management as per organisational policy.
  • Maintain accurate progress notes, incident reports, and communication records.
  • Implement individualised care plans and work collaboratively with allied health professionals.
  • Support participants to engage in social, community, and recreational activities.
  • Maintain a clean, safe, and supportive environment at all times.
  • Adhere to occupational health and safety standards

👩‍⚕️ Qualifications & Experience Required

Essential:

  • Certificate III in Individual Support, Disability, or Aged Care.
  • Demonstrated experience of supporting individuals with Huntington’s Disease, neurological conditions, or complex behaviours.
  • At least 2 years of demonstrated experience.
  • Knowledge on the implementation of PBS strategies.
  • Experience in mealtime management (texture modifications, aspiration prevention).
  • Medication administration competency and experience.
  • First Aid (HLTAID011) and CPR (HLTAID009) – current and valid.
  • Manual Handling and Infection Control training (completed within the last 12 months).
  • NDIS Worker Screening Check (or ability to obtain).
  • Australian Driver’s Licence and reliable, insured vehicle.
  • Completion of NDIS Workers’ Orientation Module
  • COVID-19 vaccination evidence (as per current WA Health requirements).

Desirable:

  • Additional qualifications or training in Behaviour Support, Dementia Care, or Mental Health.
  • Experience in supported independent living (SIL) or residential disability settings.
  • Strong understanding of Huntington’s Disease progression, neurological conditions, communication changes, and emotional regulation.
