About the Role

At Baldwin Living, we are committed to supporting older Australians to live independently, safely, and with dignity in their own homes. We are seeking a compassionate, organised, and experienced Care Partner to join our growing HomeServe team.

This remote role offers the opportunity to play a key part in coordinating and managing high-quality home care services while building meaningful relationships with participants, carers, and families.

You will support participants through person-centred care planning, ongoing assessments, service coordination, and regular communication to ensure their changing needs are understood and met. Key Responsibilities

  • Develop individualised care plans in partnership with participants, based on their assessed needs, goals, preferences, and funding arrangements.
  • Conduct ongoing reviews and reassessments, adjusting care plans to support changing needs and circumstances.
  • Apply for AT-HM funding as required.
  • Complete risk assessments while promoting participant choice and dignity of risk principles.
  • Effectively manage a caseload of participants, including service coordination, budget monitoring, and roster oversight.
  • Build and maintain strong relationships with participants, families, care staff, and other members of care teams.
  • Ensure services are delivered in accordance with the strengthened Aged Care Standards, organisational policies, and clinical governance requirements.
  • Identify, document, and escalate incidents, risks, and concerns in line with organisational processes.
  • Support participants to maximise the value of their Support at Home program funding and achieve their wellbeing goals.

While the role is primarily performed remotely, the successful candidate will be required to attend the homes of local participants when visits are needed and travel to Baldwin Living’s Victoria villages from time to time.

About You

To be successful in this role, you will bring:

  • At least 2 years’ experience in the Home Care Package or Support at Home space.
  • A strong understanding of the Support at Home Program, the new Aged Care Act, and contemporary person-centred care principles.
  • Excellent communication, relationship-building, and stakeholder management skills.
  • Strong organisational skills with the ability to manage competing priorities and caseloads effectively.
  • IT skills including confidence using care management systems, and Microsoft Office applications.
  • Sound problem-solving skills and the ability to work independently, confidently, and proactively in a remote environment.
  • Willingness and ability to attend local participant home visits as required and travel to Baldwin Living’s Victoria villages from time to time.
  • A current Australian Driver’s Licence and access to a fully insured vehicle.
  • Current Police Clearance or willingness to obtain.
