Description

SumMake a Real Impact in Your Community – Join Us as a Health Improvement Practitioner!

Are you a passionate health professional who wants to make a genuine difference in people’s lives? We’re looking for Health Improvement Practitioners (HIPs) to join our team working within General Practices across the Wellington, Porirua, Kāpiti and Hutt Valley regions. Some remote/virtual work opportunities will also be available.

This is a highly rewarding, patient-facing role where you’ll support both mental and physical wellbeing, making a real impact for individuals, whānau, and the wider community.

About the role

As a Health Improvement Practitioner, you’ll work directly with enrolled patients using the Behavioural Health Consultancy (BHC) model, providing brief, evidence‑based interventions in a primary care setting. You’ll be a key link between patients, General Practice teams, and the wider health and community network – helping people access the right support, at the right time, in the setting that works best for them.

No two days are the same. Your work may include:

  • Delivering brief evidence-based psychological interventions
  • Facilitating evidence-based group programmes to promote health and wellbeing
  • Working alongside GPs and multidisciplinary primary care teams
  • Supporting awareness and early intervention for mental health and addiction
  • Developing and strengthening group pathways to meet community needs

Comprehensive on-the-job training and coaching will be provided.

You’ll ideally have:

  • A qualification that enables you to provide therapeutic support
  • Registration that enables and endorses practice in mental health and addictions
  • Clinical experience working with clients
  • A strong understanding of evidence-based therapeutic models, particularly brief interventions
  • Proficiency in the Behavioural Health Consultancy (BHC) model.
  • A creative, solutions-focused approach and passion for talking therapies
  • The ability to work independently while being an integral part of a primary health care team
  • Confidence and capability in working with Māori and Pacific populations
