Description

Tū Ora Compass Health are seeking a Hutt-based Access and Choice Health Coach motivated to make a positive difference in people’s lives.

This is a Permanent Position, full-time based in the Hutt Valley as a regular team member, helping patients gain the knowledge, skills, tools, and confidence to become active participants in their own care and reach self-identified goals.

 Working collaboratively with GPs, Nurses, Health Improvement Practitioners and Community Support Workers to develop active partnerships with patients, the Health Coach will motivate them to make positive lifestyle changes, reduce high risk behaviours, manage stress, and improve their health and wellbeing.  

Our ideal candidate will come from a health or well-being background and be flexible, resilient, empathetic and connected to the community.

 We are seeking applicants with the following skills:

  • A willingness to embrace new ways of working
  • The ability to work with diverse patient and staff population
  • Basic knowledge of general practice, long term conditions and common mental health concerns
  • Demonstrated ability to build trust and engage effectively with people who may feel hesitant or disengaged from the healthcare system, using a respectful, non-judgemental and patient-centred approached
  • Knowledge of Te Tiriti of Waitangi principles and a commitment to Māori health equity
  • Excellent interpersonal and communication skills
  • Excellent planning and organisational skills
  • A full and current NZ driver’s license

 

For the successful applicant a nationally recognised Health Coach training will be provided with ongoing support.
