About the Role
We are seeking 3 Hauora Navigators to join our Hauora team. These roles
are dedicated to supporting whānau to navigate health, mental health, and
social service systems with confidence and dignity. You will play a key role in
advocating for Māori and Pasifika whānau, ensuring their voices are heard and
their needs are met across services. This role requires strong knowledge of
systems, culturally grounded practice, and the ability to build trusted
relationships.

community-based care

• Mental Health Practitioners
• Qualified Health and Wellbeing community workers
Key Responsibilities
• Support and walk alongside whānau to achieve their hauora
goals
• Navigate health, mental health, and social service systems
and processes
• Advocate strongly for Māori and Pasifika whānau within
services
• Conduct and contribute to hauora and mental health
assessments
Coordinate
and deliver Community based clinics, School based clinics for general health
support
• Support access to appropriate assessment and referral
pathways
• Develop and coordinate whānau-centred care and wellbeing
plans
• Build and maintain relationships with community providers
and services
• Deliver culturally safe, strengths-based support
What We’re Looking For
• Relevant qualification in health, mental health, social
work, or wellbeing
• Minimum 2 years experience in a related field
• Strong understanding of health and mental health systems,
services, and processes
• Knowledge of mental health and wellbeing assessment
pathways
• Demonstrated experience in, or a strong willingness to
engage in, Māori and Pasifika advocacy
• Understanding and application of Te Tiriti o Waitangi in
practice
• Ability to engage effectively with whānau using a
holistic, culturally grounded approach
• Strong relationship-building and communication skills
Desirable
• Professional registration where applicable
• Experience in primary care, community health, or mental
health services
• Knowledge of kaupapa Māori and Pasifika models of care
