This role is located in our Putaruru office and supports community across South Waikato.

Te tūranga | The role

 

We are seeking a compassionate and skilled Community Nurse – Chronic Care & Kaumātua Services to join our Tīwai Hauora team. This role provides integrated, community-based nursing care for kaumātua and whānau living with chronic conditions. You will deliver care beyond traditional clinical settings — including marae, whānau homes, and community clinics — supporting whānau to improve quality of life, independence, and long-term health outcomes.

Reporting to the Clinical Nurse Leader: Chronic Care & Kaumātua Services, you will hold an appropriate caseload and work within a multidisciplinary, kaupapa Māori environment.

 

You will:

  • Deliver comprehensive, culturally responsive health assessments and care planning
  • Coordinate and deliver chronic disease management (e.g. diabetes, cardiovascular, respiratory conditions)
  • Provide health education, screening, medication support, and preventative care
  • Coordinate clinical health clinics and support our weekly Kaumātua programme
  • Support post-hospital discharge planning and continuity of care
  • Work in partnership with whānau, GPs, hospitals, community providers, and internal teams
  • Maintain accurate clinical documentation and contribute to service reporting
  • Promote whānau capacity-building, self-management, and wellbeing

 About You:

  • Bachelor of Nursing with a current Annual Practising Certificate
  • Registration with the Nursing Council of New Zealand
  • Experience or strong interest in community nursing and chronic care
  • Understanding and application of Māori models of health
  • Commitment to Te Tiriti o Waitangi principles and kaupapa Māori values.
  • Basic te reo Māori and understanding of tikanga
  • Experience working with iwi or Māori organisations is highly valued
  • Strong relationship-building, organisation, and clinical judgement skills
  • Excellent communication skills to engage with a wide range of whānau.
  • A full NZ Class 1 driver licence and willingness to undergo Police vetting.
  • Experience in supporting team leadership is desirable.
