About the Role
We are seeking a compassionate and motivated Support Worker to support clients in the community. In this role, you will provide support to individuals living with a disability in our community.
Key Responsibilities
- Support clients to develop daily living and functional skills
- Provide emotional support, guidance, and problem-solving strategies
- Maintain accurate case notes and monthly reports
- Monitor client progress and follow up on referrals
- Work collaboratively with families, service providers, and stakeholders
Mandatory Requirements
- Experience working with diverse and vulnerable populations
- Excellent communication and interpersonal skills
- Ability to advocate for clients and support access to services
- Experience or willingness to facilitate group programs
- Current NDIS Worker Screening Check
- National Police Clearance
- Current Driver’s Licence
- Working With Children Check (WA)
Desirable
- Experience working in an Aboriginal Community Controlled Organisation
- Certificate IV in Community Services or related field preferred.
- Relevant experience highly sought.
