About the role:

As a Community Support Officer, working within the Social, Emotional, Wellbeing (SEWB) Team. You will provide practical, culturally appropriate and client-focused support to individuals and families. You will work closely with clients, health professionals, government agencies and community organisations to help clients navigate services and achieve positive outcomes.

This is a full-time position suited to someone who is compassionate, respectful and enjoys supporting people.

Responsibilities of the role

• ·Providing client support through walk-ins, phone contact, outreach and follow-up.
• Advocating for clients and assisting them to access services including housing, Centrelink, mental health, family and community services.
• Referring clients to appropriate internal and external health and community services.
• Supporting clients who may be involved with the court system through appropriate referrals and assistance.
• Supporting clients with housing, family services, Centrelink and other practical needs.
• Providing outreach support, including visits to refuges where required.
• Supporting families and friends of clients undertaking detoxification or rehabilitation.
• Assisting clients to develop practical daily living skills, including budgeting, confidence building and drug and alcohol awareness.
• Providing health education, health promotion and preventative health information to clients, families and the community.
• Participating in community events, workshops and health promotion activities.
• Maintaining accurate and confidential client records and documentation.
• Working collaboratively with the Social and Emotional Wellbeing and Practice teams.
• Participating in team meetings, case conferencing, training, quality improvement and community activities.
• Contributing to a safe, respectful and supportive workplace.

 What is needed for the role or About You

• We are looking for someone who is approachable, reliable and genuinely committed to supporting Aboriginal people and communities.
• Strong communication and interpersonal skills, with the ability to work effectively with adults, children, clinicians and a broad range of community and service providers.
• The ability to work independently, take initiative and manage competing priorities.
• The ability to provide opportunistic health education and health promotion to clients.
• Basic computer skills, including client data entry and email communication.
• A strong commitment to confidentiality, professionalism and client-centred practice.
• Previous experience working within an Aboriginal Community Controlled Organisation desirable.
• Experience working in community services, health, social and emotional wellbeing, family services or a related field preferred.

Licences, Clearance and/or Certificates.

• National Police Clearance
• WA WWC
• Basic first aid and CPR certification (or willingness to obtain)
• Valid WA Driver’s Licence
• Australian Right to Work
