Your New Role:

 As the Hospital in the Home (HITH) Unit Manager, you will serve as the clinical and operational cornerstone of our HITH service. This pivotal position requires a seamless blend of advanced clinical expertise and strategic leadership to ensure the delivery of safe, efficient, and high-impact patient care. Furthermore, you will play an essential role in driving our integrated service approach across Palliative Care at Home and Virtual Health portfolios.

Your clinical judgment will guide our multidisciplinary team, your operational acumen will ensure seamless daily workflows, and your leadership will foster the growth of this vital community service.

What You’ll Do:

  • Provide Clinical Leadership: Oversee and guide the clinical care provided by our HITH nursing team ensuring it meets the highest.
  • Solve Complex Challenges: Manage clinical risk, coordinate critical medication supplies, and respond to incidents to ensure unwavering patient safety.
  • Drive Operational Excellence: Create and publish efficient staff rosters, manage resources wisely, and contribute to systems that make our service run smoothly.
  • Be a Key Connector: Build strong, trusted relationships with referring hospital teams, specialists, patients, and families, acting as the professional face of our HITH service.
  • Measure & Improve: Track key performance indicators like patient satisfaction and readmission rates, using data to continuously enhance our care and impact.

What We’re Looking For:

  • A Seasoned Clinician: A Registered Nurse with AHPRA registration and a strong background (5+ years)
  • A Natural Leader: Experience as a Nurse Unit Manager or similar leadership role is highly regarded, but we will support a high-potential nurse ready to step up.
  • An Operational Problem-Solver: You’re organised, tech-savvy, and can expertly manage competing priorities like rostering, supply costs, and patient needs.
