As we scale, we’re growing our Care team – and we’re looking for experienced Senior Care Partners to join us.

 

Role Overview 

As a Senior Care Partner, you’ll manage a large portfolio of clients and be the person they rely on to keep their care running smoothly. You’ll handle phone-based client interactions to resolve queries, arrange services and provide ongoing support, managing a high volume of inbound and outbound calls while keeping the quality of each conversation high.

What You’ll Do 

  • Manage a large portfolio of clients, conducting regular check-in calls to maintain strong relationships and identify changing needs 
  • Manage a high volume of inbound and outbound calls, responding promptly to assigned and missed contacts 
  • Complete client onboarding, including introductions and initial setup of services 
  • Review and action on-hold invoices within required timeframes 
  • Monitor client budgets and service plans, tracking expenditure to prevent overspend and ensure charges align with eligible funding 
  • Monitor client wellbeing and report incidents in line with SIRS requirements, following up on incident management plans as directed by clinical staff 
  • Maintain accurate, up-to-date client records and detailed case notes across multiple systems 
  • Provide guidance and mentorship to Care Partners, and support the Team Leader with case consultations and coverage during absences 
  • Assist with onboarding and training of new team members, promoting collaboration and knowledge sharing across the team 

What You’ll Bring 

  • At least 1-2 year of experience in care coordination, case management, or NDIS support services 
  • Proven ability to manage a high-volume client portfolio 
  • Strong communication skills, with the ability to build rapport with diverse clients and navigate complex or sensitive situations 
  • Highly developed organisational and time management skills, with the ability to prioritise competing demands 
  • Confident using CRM and client management systems, Microsoft 365 and new technology 
  • Solid understanding of the Support at Home framework, including funding structures and service delivery requirements 
  • Demonstrated ability to mentor or coach others and step up when the team needs it 
  • A warm, resilient and collaborative approach, and a commitment to professionalism 
  • Experience in aged care, health, disability or community services is desirable
