This office-based clinical leadership role combines your clinical expertise with modern health tech to improve care quality, manage clinical risk, and support best practice across our Home Care services. You’ll provide clinical supervision and expert guidance to Team Leaders and Care Partners, helping drive safer outcomes and continuous improvement.

What you’ll do
- Provide clinical supervision and guidance to Team Leaders and Care Partners
- Act as the escalation point for complex clinical cases and risk management matters
- Support incident management in line with SIRS and regulatory requirements
- Strengthen clinical capability through mentoring and targeted training
- Ensure care planning aligns with the Aged Care Act and Quality Standards
- Partner with Clinical Governance to drive continuous improvement
- Contribute to a culture of safety, accountability and clinical excellence
- This role has no direct reports but holds significant clinical influence and leadership responsibility.
What you’ll bring
- Current registration as a Registered Nurse with Australian Health Practitioner Regulation Agency.
- Demonstrated experience in general medical nursing with strong assessment, clinical decision-making and care coordination skills.
- Experience managing clients with chronic and complex health conditions across community, hospital or aged care settings.
- Sound knowledge of gerontology, healthy ageing, frailty, dementia, cognitive impairment and the clinical issues affecting older people.
- Experience in aged care, community nursing, care coordination or chronic disease management is highly regarded.
- General medical nursing experience with the ability to assess, monitor and manage clients with chronic and complex health conditions.
- Demonstrated experience in the care of older people, including chronic disease management, gerontology, frailty, dementia and cognitive impairment.
- Experience undertaking comprehensive clinical assessments, recognising deterioration, coordinating multidisciplinary care, and supporting evidence-based clinical decision-making.
- Highly developed communication skills with the ability to provide clinical advice and support to clients, carers, general practitioners and other healthcare providers in a remote care environment.
