Summary:

The Support Coordination Team Leader will play a pivotal role in enhancing the quality of social care services in Ballarat and Bendigo. This position is essential for leading a dedicated team of support coordinators, ensuring that clients receive tailored support to navigate their care options effectively. The Team Leader will foster a collaborative environment, driving best practices and ensuring compliance with relevant regulations, ultimately contributing to the well-being of the community.

Responsibilities:

  • Lead and manage a team of support coordinators, providing guidance and support to ensure high-quality service delivery.
  • Develop and implement individual support plans in collaboration with clients and their families.
  • Monitor and evaluate the effectiveness of support services, making adjustments as necessary to meet client needs.
  • Ensure compliance with all relevant legislation and organisational policies.
  • Facilitate training and professional development opportunities for team members.
  • Build and maintain strong relationships with clients, families, and external stakeholders.
  • Conduct regular team meetings to discuss progress, challenges, and strategies for improvement.
  • Prepare reports and documentation as required for internal and external stakeholders.

Qualifications:

  • Bachelor’s degree in Social Work, Psychology, or a related field.
  • A minimum of 5 years’ experience in social care or support coordination, with at least 2 years in a leadership role.
  • Strong understanding of the National Disability Insurance Scheme (NDIS) and relevant legislation.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
  • Proven ability to lead and motivate a team, fostering a positive and productive work environment.
  • Strong organisational skills and the ability to manage multiple priorities effectively.
  • Proficiency in using case management software and Microsoft Office Suite.
