Description

If you have a background in a leadership role within the NDIS Support/SIL environment and some Case Management/ Service Coordinator experience, this might be your next career opportunity. Our client is seeking to add to their team of Service Coordinators in the Northern Rivers Region.

Your primary responsibility will be the coordination of safe, high-quality, person -centred disability services that promote participant choice, independence, and wellbeing while ensuring full compliance with the NDIS Practice Standards and organisational governance frameworks.

You will act as the operational link between participants, frontline staff, families, and external stakeholders to ensure responsive, consistent, and outcome-focused service delivery.

Key Responsibilities

  • Coordinate delivery of NDIS-funded supports in line with participant goals, plans, and service agreements
  • Collaborate with participants, families, guardians, and allied health professionals to develop and review individualised support plans
  • Ensure Behaviour Support Plans are communicated, implemented correctly, and supported through staff training
  • Monitor participant wellbeing, service quality, and satisfaction, implementing adjustments where required
  • Provide day-to-day operational guidance, supervision, and support to frontline staff
  • Support roster coordination, workforce performance, onboarding, and competency development
  • Ensure accurate documentation including shift notes, incident reports, risk assessments, and participant records
  • Support incident response, safeguarding, restrictive practice compliance, and duty of care obligations
  • Participate in audits, quality improvement activities, and organisational compliance processes
  • Maintain professional stakeholder communication and attend case conferences, reviews, and planning meeting.

About You

  • Demonstrated experience working within the NDIS or disability services sector
  • Minimum 1-2 years of experience in NDIS or disability services is required
  • Strong understanding of NDIS Practice Standards, safeguarding, and risk management
  • Experience coordinating support workers and service delivery in SIL, STA, or complex care environments
  • Excellent organisational, communication, and documentation skills
  • Ability to manage competing priorities while maintaining high professional and ethical standards
  • Relevant qualification in Disability, Community Services, or related field (Certificate IV, Diploma, or higher preferred)
