The Co-Group is seeking a dedicated and compassionate Care Partner to join our provider team in Tea Gardens, supporting clients across the Myall Coast region.
As a Care Partner, you will deliver coordinated, person-centred care that supports clients to achieve their individual goals while promoting independence, dignity, wellbeing, and quality of life. You will work collaboratively with clients, families, and multidisciplinary teams to ensure high-quality care and positive outcomes.
Remuneration for this position is negotiable and will be determined based on the successful candidate’s qualifications, skills, experience and suitability for the role.
Key Responsibilities
- Coordinate and deliver person-centred care and support in partnership with clients, families, and the broader care team.
- Assist in the development, implementation, and review of individual care plans.
- Conduct client assessments and monitor changing support needs.
- Maintain accurate, timely, and confidential client records and documentation.
- Promote client choice, independence, dignity, and overall wellbeing.
- Provide guidance and support to care staff as required.
- Assist with staff induction, orientation, and ongoing development.
- Communicate any changes in client needs, risks, or concerns to relevant senior staff.
- Participate in quality improvement initiatives and continuous improvement activities.
- Undertake other duties consistent with the role as directed by management.

Qualifications and Experience
- Diploma or equivalent qualification in Individual Support, Community Services, Aged Care, Disability, or a related field.
- Previous experience in a care, support, aged care, disability, or community services environment.
- Experience contributing to care planning, client assessment, and service delivery is highly regarded.
