Description

The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West of Western Australia. SWAMS delivers a wide range of primary health and community programs and has a strong growth strategy through partnership opportunities and future community development.

South West Aboriginal Medical Service is seeking a caring, community minded Care Navigator to support Aboriginal people from cancer screening and investigation through to diagnosis.

In this role, you will walk alongside people and their families, helping them understand appointments, next steps, overcome barriers to care, and feel culturally safe and supported throughout their journey. You will also work closely with community, Elders, health services and project partners to improve access to culturally safe cancer care.

This role is a Full-time role and is budgeted until June 2028. SWAMS is a family friendly employer that is open to discussing what hours works best for you and your situation. This role is based in Bunbury.

This is a varied role that includes but is not limited too:

  • ·Attend and provide culturally safe facilitation support at co-design workshops, Aboriginal community consultations, stakeholder meetings, information sessions and other approved project engagement activities.
  • ·Support inclusive participation by welcoming participants explaining the purpose and activities in accessible language, promoting discussion where appropriate and helping participants contribute safely.
  • ·Listen to community and stakeholder experience, identify key themes, barriers and information needs, and provide structured feedback to the Project Officer and Health Promotion Officer.
  • ·Work with the Project Officer on workshop and consultation preparation and follow-up. The Project Officer remains responsible for invitations, bookings, agendas, papers, minutes, stipend payments, records and event administration. Maintain active working relationships with SWAMS teams, SMHS, Cancer Council WA, GPs, specialists, Aboriginal Health Liaison Officers and other approved project or service partners
  • Participate in relevant Aboriginal MDT meetings and case discussions, represent patient preferences with consent, and raise cultural safety or access considerations.

Essential Criteria

  • ·Aboriginal or Torres Strait Islander descent (Section 50(d) Equal Opportunity Act)
  • ·National Police Clearance
  • ·Current WA C or CA Class driver’s licence
  • ·Working with Children Check
  • Ability to travel to project meetings or workshops as required 

Desirable Criteria

  • ·Experience working in Aboriginal, cancer care, chronic disease, research, community engagement, health promotion or service navigation 
  • ·Knowledge of cancer screening, cancer care pathways, multidisciplinary team models or patient navigation 
  • ·Experience working with Aboriginal governance groups, advisory panels, Elders, community and/or people with lived experience 
  • Experience in delivering culturally adapted information resources or health promotion materials.
