Summary:
We are seeking a dedicated and experienced Senior Practice Nurse to join our medical team in Sale, Victoria. This permanent, full-time role is essential for providing high-quality nursing care and support to our patients. The Senior Practice Nurse will play a pivotal role in enhancing patient outcomes and ensuring the smooth operation of our practice, contributing to the overall health and wellbeing of the community.
Responsibilities:
Provide clinical nursing services to clients attending the clinics including but not limited to:
- Wound Care
- Venepuncture
- Electrocardiogram and Holter Monitor
- Understanding of Victorian Immunisation Schedule, the National Immunisation Program eligibility, funding and administration of immunisations.
- Implementation and support of GPs in preventative health and disease management through health assessments including but not limited to; Aboriginal and Torres Strait Islander Health Check, GP Chronic Condition Management Plan, Heart Health Check.
- Supporting and assisting GPs with procedures.
- Understanding of health screening including but not limited to; Cancer (Cervical, Bowel, Lung, Breast), Metabolic Health, General patient observations.
- Health promotion
Knowledge and maintenance of and reminder system
Provide a link between services and patients, through networking and liaising with:
- Local hospitals and GP clinics
- Home Visiting Services
- Aged Care Assessment Teams
- Allied Health Providers
- GPs / Specialists
- NDIS Service Providers
- Services Australia
- Department of Health
Management of the clinical environment by assisting the service to meet relevant standards and legislative requirements in:
Oversite of stock control for medical supplies and immunisations
Oversite of signed monthly clinical checks, including; Expirations and stock, Medications, Dr’s Bag
Clinic stock, Immunisations, Emergency Trolley, Blood Glucose Meter Calibration, Automated External Defibrillator, Oxygen, Oversite of GP consult room restock, Infection control, Cold chain management, Records management, Occupational health and safety, Accreditation processes.
Provide support to Practice Management in the following areas:
- Education and support of clinical staff
- Upload of eHealth Summaries to MyHealth Record for at risk of hospitalisation clients
- Monitoring clinical software holding file for urgent results and hospitalisation discharge summaries for GPs to action
- Annual education monitoring and competencies of clinical staff
- Computer skills-appointments / data entry
- Scheduling and documentation of routine maintenance or repair of medical equipment
- Knowledge of MBS item numbers
Qualifications:
- Registered Nurse with a current Australian Health Practitioner Regulation Agency (AHPRA) registration.
- Bachelor’s degree in Nursing.
- Minimum of 5 years’ experience in a clinical nursing role, preferably in a primary care setting.
- Strong clinical assessment and decision-making skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively both independently and as part of a team.
