Summary:

The HACC-PYP program supports Aboriginal and Torres Strait Islander people aged from birth to under 50 years to improve their health and wellbeing, while maintaining or regaining their independence to live safely in their homes and actively participate in their community.

The HACC-PYP Assessment Officer will undertake comprehensive assessments, care planning, and coordination of supports to meet the client’s identified needs. This role ensures clients receive culturally appropriate, person-centred support that helps maintain independence and engagement within the community.

This includes supporting younger people with a disability to access government funded services or link into other government programs such as the NDIS appropriate to their needs.

Responsibilities:

Assessment & Client Support

• Conduct comprehensive, culturally safe assessments for HACC-PYP clients.
• Identify individual support needs, goals, and care plans in consultation with clients and families.
• Collaborate with clients, families, and carers to develop tailored service plans.

Coordination & Referrals

• Facilitate referrals to internal services or external providers as appropriate.
• Work closely with allied health, medical, and community service providers.
• Maintain client records and documentation in accordance with organisational and funding requirements.

Community Engagement

• Build strong relationships with Aboriginal and Torres Strait Islander clients and communities.
• Promote awareness of HACC-PYP programs within the community.
• Ensure services are delivered in a culturally safe and respectful manner.
• General stakeholder engagement.

Compliance & Reporting

• Maintain accurate and timely records in line with funding, privacy, and confidentiality requirements.
• Contribute to reports for internal use and funding bodies as required.
• Comply with relevant legislation, standards, and organisational policies.

Qualifications:

• Relevant qualifications in social work, allied health, community services, nursing or Aboriginal health (or equivalent experience) desirable, but not essential.
• Experience working with Aboriginal and Torres Strait Islander communities is highly desirable.
