Our mission is to improve the lives of people with renal failure, reunite families and reduce the impact of kidney disease in our communities. We are run by Aboriginal people for Aboriginal people and work to provide culturally appropriate health, aged care and general support services in regional and remote communities, helping people to get home to country and family.

About the Role

The Chronic Disease Care Coordinator is responsible for offering extra support to high-risk patients with Chronic Diseases. The objective is to enhance their access to healthcare services and promote positive health outcomes. This role includes helping patients set goals, identify the necessary health and support services to achieve those goals, navigate the healthcare system, and advocate for their individual needs.

Key responsibilities

  • Collaborate with internal and external stakeholders to support continuity of culturally safe, holistic and high-quality patient care aligned with Purple House values and strategic goals.
  • Work in partnership with patients, families and carers, where appropriate, to identify individual needs, strengths and priorities and develop person-centred, strengths-based care plans aligned with each patient’s goals.
  • Coordinate care for patients living with multiple and complex chronic conditions, including scheduling of appointments and management of healthcare services.
  • Advocate for patients and promote preventive healthcare, screening, health education and supported self-management to assist patients in understanding and managing their health conditions.
  • Provide flexible, responsive and accessible care that recognises the circumstances, needs and preferences of individual patients.
  • Maintain accurate, timely and comprehensive patient records and use follow-up systems to ensure healthcare needs are addressed in a timely manner, including proactive follow-up of missed appointments and patients at risk of disengaging from care.
  • Recognise the impact of social and cultural determinants of health and work closely with the Purple House Malpa team and other relevant services to address barriers to care and support holistic patient goals.
  • Deliver care in a culturally safe and trauma-informed manner, including supporting informed consent, appropriate interpreter use, privacy, confidentiality and patient choice.
  • Comply with all relevant legislation, professional standards, organisational policies and clinical governance requirements, including infection prevention and control, safeguarding, privacy, incident reporting and workplace health and safety.
  • Participate in clinical audits, outcome monitoring, service evaluation and continuous quality-improvement activities to strengthen patient care and service delivery.
  • Perform other duties within the employee’s qualifications, competence, authorised scope of practice and classification level, as reasonably directed.
