Summary:
The Care Coordinator will be responsible for coordinating patient care services, advocating for patient needs, and ensuring effective communication between healthcare professionals, patients, and their families or caregivers.
Responsibilities:
- Coordinate and manage patient care services based on individual needs and resources
- Conduct comprehensive assessments and develop individual care plans
- Collaborate with healthcare professionals and other care coordinators
- Advocate for patients’ access to necessary medical services and resources
- Schedule and monitor appointments, referrals, and care transitions
- Communicate with patients and families to provide guidance and education on disease management and self-care strategies
- Maintain accurate patient records and care plans
- Participate in team meetings and quality improvement initiatives
Qualifications:
- Bachelor’s degree in Healthcare Administration, Nursing, Social Work or related field
- Minimum of two years of experience in healthcare or care coordination
- Knowledge of healthcare delivery systems and care coordination techniques
- Excellent organizational, communication, and interpersonal skills
- Ability to work independently or as part of a team in a fast-paced environment
- Strong analytical and problem-solving skills
- Commitment to patient-centered care and empathy for patient needs
- Proficiency in medical terminology, electronic health records, and Microsoft Office Suite
