Position Title: Exercise Physiologist
Location: Busselton

Position Overview
Provide evidence-based assessment, exercise programs and ongoing support to people with chronic conditions, improving health outcomes, function, and self-management through collaborative care.
Key Responsibilities
• Work as a member of the Integrated Chronic Disease Care (ICDC) team.
• Provide high quality assessment and develop individualised physical activity programs for clients with targeted chronic disease (COPD, asthma, diabetes and cardiac conditions).
• Provide follow-up care and ongoing monitoring of clients’ progress as indicated.
• Deliver patient-centred services that improve health literacy and promote self-management.
• In conjunction with fellow ICDC team members develop, plan and implement group- based health promotion and education sessions promoting chronic disease self-management.
• Ensure all services are delivered in accordance with professional standards adhering to best practice guidelines and remain within the exercise physiology scope of practice.
• Engage in multidisciplinary care planning and case conferences as needed.
• Ensure client services are delivered in an integrated and collaborative manner communicating and providing feedback to other health professionals involved in delivering services to clients of the ICDC program e.g. General Practitioners, Practice Nurses, WA Country Health Service staff, allied health and community support staff.
• Provide consultative advice and support to other GP down south staff members.
• Develop and maintain strong collaborative relationships with health partners and service providers particularly WACHS and General Practice staff.
To be considered for this role, you must meet the following criteria:
• Tertiary qualification in Exercise Physiology
• Accreditation as an Accredited Exercise Physiologist (AEP) with current membership and accreditation through Exercise and Sports Science Australia (ESSA).
• Minimum 2 years’ experience as an Exercise Physiologist, ideally in primary health or community settings
- Current Western Australian Driver’s Licence
- National Police Clearance
- A Working with Children Check (WWC)
- Pre-Employment Health Assessment
