Taylored Support is looking for an experienced and motivated Support Coordinator to join our team on an urgent basis.
This is an exciting opportunity for someone who is passionate about supporting NDIS participants to achieve their goals, build independence and access the right supports and services.
The successful candidate will be required to travel across Western Sydney and Central Western Sydney, supporting participants across areas including Penrith and surrounding suburbs through to Lithgow.
About the Role
As a Support Coordinator, you will work closely with NDIS participants, families, support providers and other stakeholders to ensure participants receive appropriate, high-quality and person-centred supports.
Your responsibilities will include:
- Coordinating and monitoring participant supports and services
- Building strong relationships with participants, families and service providers
- Supporting participants to understand and utilise their NDIS plans
- Connecting participants with appropriate community and mainstream services
- Developing and maintaining service agreements and participant documentation
- Monitoring participant outcomes, goals and support needs
- Maintaining accurate and up-to-date case notes and records
- Ensuring all documentation and services meet NDIS requirements
- Liaising with internal teams and external providers
- Identifying opportunities to improve participant outcomes and service delivery
- Managing a caseload effectively while maintaining a high standard of participant care
About You
We are looking for someone who is confident, organised and highly experienced in NDIS Support Coordination and can work independently while maintaining strong communication with participants and stakeholders.
To be successful, you will have:
- Minimum 3 years of experience in Support Coordination
- A strong understanding of the NDIS framework, compliance requirements and participant rights
- Strong knowledge of NDIS service agreements and service delivery requirements
- Excellent communication and relationship-building skills
- Strong organisational and time-management skills
- The ability to manage a caseload and work autonomously
- A professional and participant-focused approach
- A current driver’s licence and reliable vehicle
- The willingness and ability to travel throughout Western Sydney and Central Western Sydney, including Penrith, surrounding areas and Lithgow
