About the Role

We are seeking an experienced Case Manager to deliver support across our HACC-PYP, Linkages and Transitional Housing programs.

This diverse and rewarding role supports people experiencing complex mental health, housing and social challenges to build independence, strengthen community connections, access stable housing and achieve their personal goals.

You will work collaboratively with participants, carers and service providers to undertake assessments, develop goal-directed support plans, coordinate services, advocate for client outcomes and facilitate transitions to longer-term supports such as the NDIS, My Aged Care and social housing pathways.

This role offers a balance of case management, care coordination, tenancy and housing support, and community engagement within a supportive multidisciplinary team.

Key Responsibilities

• Conduct holistic assessments and develop person-centred support plans.
• Coordinate services and build strong partnerships with health, housing and community providers.
• Support individuals with complex needs to achieve their recovery, wellbeing and housing goals.
• Assist participants to access and transition to appropriate long-term support services.
• Provide tenancy education, housing advocacy and support to sustain accommodation.
• Complete risk assessments, safety planning and crisis interventions where required.
• Maintain accurate case notes, reports and program data.
• Manage a caseload while achieving program outcomes and targets.
• Participate in supervision, team meetings and ongoing professional development.

About You

You will be passionate about supporting people experiencing mental health challenges, housing instability and social disadvantage and have:

• A qualification in Community Services, Social Work, Psychology, Occupational Therapy, Nursing or a related field (Diploma minimum).
• Experience in case management, care coordination, housing, homelessness, mental health or community services.
• Strong understanding of recovery-oriented, trauma-informed and person-centred practice.
• Experience supporting people with complex needs and working collaboratively with carers and external services.
• Knowledge of housing pathways, tenancy responsibilities and relevant support systems.
• Well-developed assessment, advocacy and relationship-building skills.
• Excellent written, organisational and time management skills.
