Description

Summary:

As our NDIS Support Coordinator based in Brisbane, you will provide support to people in Brisbane, Rockhampton, Mackay and Canberra. The current caseload allows for a total of 20 hours support (including indirect support) per week however we encourage you to promote the service to grow the number of Participants being supported and therefore your work hours.

Pay rate is $51 per hour.

You will play a vital role in assisting individuals with disabilities and their Representatives to navigate the National Disability Insurance Scheme (NDIS). This part-time position is essential for empowering Participants to achieve their goals and enhance their quality of life through tailored support and guidance. Your expertise will contribute significantly to Participants understanding their plan, ensuring that Participants receive the necessary resources and assistance to work towards achieving their goals.

Responsibilities:

  • Assist Participants in understanding and accessing NDIS services and supports.
  • Coordinate with service providers to ensure the delivery of high-quality care that meets the goals as prescribed in the Participant’s NDIS plan.
  • Monitor and review Participant progress, adjusting support plans as necessary.
  • Provide advocacy and support to Participants in navigating the NDIS process.
  • Maintain accurate records and documentation of Participant interactions and progress.
  • Prepare reports when requested by the Participant/ Representative or required for reviews and changes in circumstances applications.
  • Collaborate with multidisciplinary teams to enhance service delivery.

  • Experience in a support or coordination role, preferably within the NDIS framework.
  • Strong communication and interpersonal skills.

  • Excellent organisational skills and attention to detail.
