What We’re Looking For
- Current AHPRA registration as a Registered Nurse
- Experience in support at home is highly desirable
- Minimum 3 years of clinical nursing experience, with demonstrated capability in community or aged care settings
- Strong experience in wound care, catheter care, and complex case management
- Proven clinical leadership and mentoring skills
- A current driver’s licence and access to a reliable vehicle
- Current National Police Check or willingness to obtain
- Unrestricted work rights in Australia
- Excellent communication and documentation skills
- Commitment to best practice, holistic care, and continuous improvement
- Must be available to work 30 hours per week, Monday to Friday, with flexibility to undertake shifts across both AM and PM hours.
- Must be willing to travel across the Brisbane North region
- Immediate availability to commence employment is highly desirable.

Key Responsibilities
Complex Wound Care
- Conduct thorough wound assessments (including photography, measurements, and staging)
- Develop and implement individualized wound management plans
- Evaluate dressing types, advanced therapies, and negative pressure wound options
Catheter Management
- Insert, maintain, and troubleshoot indwelling (IDC) and suprapubic (SPC) catheters
- Educate patients and carers on proper catheter care, infection prevention, and early signs of complications
Patient Assessment & Care Planning
- Perform holistic in-home assessments (covering physical, psychosocial, and environmental factors)
- Develop and review person-centered care plans in collaboration with allied health and medical teams
- Coordinate referrals to relevant health and community services
Clinical Leadership
- Mentor and support junior nurses and student placements in the community
- Lead and contribute to the development of clinical policies, procedures, and best-practice guidelines
- Participate in clinical audits and quality improvement initiatives
