Care Partnerships Lead

Business Development | Relationship Management | Aged Care & Community Care Growth

About Harwell Home Care

Harwell Home Care is a national provider of community and home-based care services, supporting older Australians to live safely, independently, and with dignity in their own homes. We deliver high-quality services across the Support at Home Program, DVA Community Nursing, and other community-based care services.

Driven by a commitment to quality, compassion, and continuous improvement, we are expanding our presence across Australia and investing in leaders who share our vision for exceptional community care.

About the Opportunity

Harwell Home Care is growing, and we are looking for a driven, relationship-led Care Partnerships Lead to help us open new doors, build meaningful referral pathways from the ground up, and expand our presence across new regions. You will work across healthcare, community, veteran and aged care networks to establish and grow strong referral relationships.

This is a true business development role for someone who loves being out in market, meeting people, creating opportunities, and turning strong relationships into real growth outcomes. This role will suit someone who is commercially minded, confident, proactive, and genuinely energised by growth.

Reporting to the Chief Innovation & Growth Officer, this role sits within our Innovation & Growth team and plays a key part in expanding Harwell’s client base and market footprint.

Key Accountabilities

  • Identify and prioritise new regions for growth, develop market entry plans, and establish Harwell’s presence in greenfield markets.
  • Proactively build relationships with referral partners across hospitals, GPs, allied health, community organisations, veteran networks and other aligned channels.
  • Own and manage a new business pipeline, including targets, conversion rates, CRM activity and growth metrics. The role requires strong pipeline discipline, regular reporting, and a clear focus on turning relationships into measurable outcomes.
  • Identify and secure strategic partnerships that create new client streams, support regional expansion, and build Harwell’s presence in the market.
  • Work closely with our Care Concierge, Marketing & Brand, and broader internal teams to ensure market insights are shared, campaigns are supported, and referral opportunities are followed through properly.

About you

You are a natural relationship builder with strong commercial instincts. You know how to open doors, build trust quickly, and turn conversations into opportunities. You may come from business development, sales, relationship management, partnerships, healthcare, aged care, community nursing, home care, DVA, hospital engagement, or a similar growth-focused background.

Most importantly, you are hungry, values-led, and genuinely motivated by the opportunity to grow a service that helps people remain supported at home and in their community.

You will bring:

  • Proven experience in business development, sales, relationship management or market expansion.
  • A strong track record of building relationships that convert into real business outcomes.
  • Confidence managing a pipeline, tracking activity, working toward targets, and reporting on performance.
  • Excellent stakeholder engagement and influencing skills.
  • Strong organisational skills and the ability to manage multiple regions, relationships and opportunities at once.
  • Confidence using CRM systems to track pipeline, performance and activity.
  • A willingness to travel regularly to meet with referral partners, attend events, build networks and establish Harwell in new markets.
  • A background in healthcare, community nursing, home care, aged care or community services would be highly advantageous.
  • Experience working with hospitals, GPs, allied health providers, DVA, discharge planners, case managers or referral-based care environments would also be highly regarded.
  • Existing networks across healthcare, hospital, community nursing, home care, allied health or aged care referral pathways would be a strong advantage.
