Family and Community Services Team

The Family and Community Services team plays a vital role in supporting Aboriginal children, young people, families, carers and community members across all stages of life.

The team delivers culturally safe, strengths-based and trauma-informed programs that focus on social, emotional, cultural, health and wellbeing outcomes. This includes providing support through advocacy, cultural planning, case management, practical assistance, early intervention, referrals and connection to family, community, Country and culture.

Role Summary

The Targeted Care Package Coordinator (TCP) works with complex Aboriginal children and young people and their families to transition into care arrangements where their needs will be better met. The TCP Coordinator will engage with young people, their Care Teams and families towards returning the child or young person home with supports or building a sustainable and secure placement for the young person in kinship or foster care, or independent/ semi-independent living.

The aim of the TCP program is first and foremost to achieve better outcomes for children and young people. The best interests of children and young people will guide all decision-making when considering care and support options, to ensure a tailored response. At the heart of the framework is the child and carer with specialist therapeutic intervention & support services wrapped around the young person to enable them to experience a sense of belonging, support to achieve and thrive with hope for their future.

Duties and Responsibilities

• Coordinate the planning, implementation and review of Targeted Care Packages for children and young people in line with DFFH requirements, organisational policies and the individual needs of each child or young person.
• Work collaboratively with DFFH Child Protection, carers, families, case managers, ACCOs, schools, health providers, therapeutic services and other stakeholders to support stable, safe and appropriate care arrangements.
• Develop, monitor and review individual TCP plans that are flexible, strengths-based, culturally safe and focused on achieving positive outcomes for the child or young person.
• Support children and young people to transition from residential care to more suitable living arrangements, or to maintain family-based, kinship, foster or alternative care arrangements where appropriate.
• Act as a central point of coordination for TCP supports, ensuring services are delivered in a timely, responsive and child-focused way.
• Monitor the use of TCP funding to ensure expenditure is appropriate, approved, clearly documented and aligned with the goals and needs identified in the package plan.
• Build strong working relationships with children, young people, carers and families using a trauma-informed, therapeutic and culturally respectful approach.
• Identify risks, placement pressures and support needs early, and work with care teams to develop practical strategies that promote stability, safety and wellbeing.
• Maintain accurate and timely documentation, including case notes, TCP plans, review reports, financial records, service agreements and DFFH reporting requirements.
• Support Aboriginal children and young people to remain connected to family, community, culture and Country, and work alongside Aboriginal services and cultural supports wherever required.
• Participate in care team meetings, professional consultations, supervision and program reviews to ensure coordinated planning and accountability.
• Contribute to continuous improvement of the TCP program by identifying service gaps, strengthening partnerships and supporting flexible, innovative responses to meet the needs of children and young people.

Qualifications, Skills, Experience and Knowledge

Essential

• Relevant Tertiary Qualifications in the Social Services Sector such as Diploma of Community Services.
• Experience working with Aboriginal children, young people and families.
• Demonstrated ability to achieve positive outcomes for young people through a strengths-based, outcomes-focused case management approach
• Ensure all service delivery delivered within the TCP program is delivered in a culturally safe, strength-based practice including all service delivery staff.
• Experience in the support and strengths-based supervision of staff and or carers.
• Knowledge and experience in therapeutic frameworks, including evaluating strategies and formulating recommendations.
• Knowledge of the Out of Home Care system and Child Protection statutory requirements
• Demonstrated ability to working collaboratively and the capacity to negotiate and liaise with staff, carers and volunteers, Aboriginal community and families, DFFH, other stakeholders
• Demonstrated understanding of, and respect for, the needs of Aboriginal children and young people including cultural safety and awareness.
• Excellent written and oral communication skills (including public speaking, presentations and facilitation skills).
• Demonstrated ability to flexibly manage competing priorities and stressful situations, monitoring own stress levels and practicing and promoting self-care strategies
• Ability to assess additional staffing resources and provide clear communication and evidence to demonstrate need to Supervisor.

Desirable

• Identifies as Aboriginal and/or Torres Strait Islander.
• Previous experience working in an Aboriginal / Torres Strait Islander organisation.

This is a 12-month contracted position and on-going employment is dependent on funding.
