Gunditjmara Health Clinic

The Health Clinic operates as a general practice, providing bulk-billed medical care, chronic disease management, health checks, and early intervention programs alongside allied and specialist services. Care is delivered through a strengths-based, holistic model that places culture at the centre of practice, supporting community members at all life stages.

The Clinic works as part of a broader wraparound service model, with strong links into Social and Emotional Wellbeing (SEWB), Family Services, Alcohol and Other Drugs (AOD), Kinship Care, Independent Living, Family Violence support, and community groups and programs. This connected approach ensures clients can access coordinated, holistic support across their medical, social, and cultural needs.

Role Summary

The Practice Nurse coordinates and delivers General Practice nursing activities, ensuring high-quality, culturally safe clinical care is provided to clients of the Health Clinic at Gunditjmara Aboriginal Cooperative (GAC). The role is responsible for the clinical nursing functions of the practice including chronic disease management, immunisation, health assessments, cannulation and iron infusions, and treatment room procedures, delivered in line with GAC’s clinical protocols and the Royal Australian College of General Practitioners (RACGP) accreditation standards.

This position directly supports the Health Clinic’s funding and accreditation obligations, including Medicare Benefits Schedule (MBS) health assessment and chronic disease targets, immunisation coverage requirements, and client care outcomes reported against GAC’s funding agreements. Success in the role is measured by the timely, accurate delivery of clinical care and compliance activity that supports these targets.

The Practice Nurse works as a member of the multidisciplinary team at the Health Clinic, working closely with GPs, GP Registrars, and Aboriginal Health Practitioners/Workers to deliver coordinated, wraparound care, and liaises with Allied Health practitioners and other programs across the organisation to support holistic client outcomes. In doing so, the role contributes directly to GAC’s mission of improving health outcomes and self-determination for the local Aboriginal community.

Qualifications, Skills, Experience and Knowledge

Essential

• Bachelor of Nursing (or equivalent) with current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (AHPRA)
• Current CPR and First Aid certification
• Experience in general practice, primary health, or community health nursing
• Ability to assess, plan, implement, and evaluate nursing care in line with accepted nursing practice and standards
• Sound knowledge of chronic disease management, immunisation schedules, and preventative health programs, including Enhanced Primary Care items
• Competency in clinical procedures relevant to general practice nursing, including wound care, venepuncture, cannulation, ECG and spirometry testing, and use of 24-hour blood pressure and ECG monitors
• Understanding of infection control principles and cold chain management
• Ability to work collaboratively within a multidisciplinary team and coordinate care with GPs, Allied Health, and Aboriginal Health Practitioners/Workers
• Excellent interpersonal and communication skills, with the ability to engage sensitively and respectfully with Aboriginal and Torres Strait Islander clients and families
• Strong organisational skills, with the ability to manage competing clinical priorities within a busy practice environment

Desirable

• Nurse Immuniser endorsement
• Qualification or training in health promotion
• Experience working with Aboriginal and Torres Strait Islander communities, or a demonstrated understanding of Aboriginal community-controlled health
• Experience using clinical and practice management software, such as Best Practice, Cubiko, and PracticeHub
