About the Role

We are seeking a passionate and community-focused Social & Emotional Wellbeing (SEWB) Link Worker to join our team on a 6-month fixed-term basis.

This is an opportunity to make a meaningful contribution within an Aboriginal Community Controlled Health Organisation, supporting clients to navigate mental health, Alcohol and Other Drugs (AOD), and broader social and emotional wellbeing services.

The SEWB Link Worker provides culturally appropriate, practical and coordinated support to clients experiencing a range of needs, including mental health and AOD concerns, grief and loss, trauma, family breakdown, domestic violence, cultural dislocation, racism and discrimination.

Key Responsibilities

• Conduct initial and ongoing client assessments using appropriate tools and frameworks, including K10 and IRIS.
• Develop individualised care plans in partnership with clients, families and relevant health and community services.
• Provide direct support and care coordination in line with the GWAHS SEWB Model of Care.
• Connect clients with appropriate mental health, AOD, primary health care, counselling and community-based services.
• Develop and maintain referral pathways across housing, employment, health, living skills, social engagement and family/community supports.
• Work closely with GWAHS GPs, nurses, clinical teams and other internal programs.
• Provide practical assistance to clients, including transport to appointments and community support programs where required.
• Maintain accurate and timely client records, case notes and program data.
• Participate in case reviews, team meetings, clinical supervision and multidisciplinary collaboration.

Essential Criteria

• Tertiary Qualification in Community Services, Mental Health, Alcohol and Other Drugs or a related field
• Strong understanding of and experience working with Aboriginal people, families and communities.
• Demonstrated cultural awareness and sensitivity to Aboriginal culture, history and contemporary issues affecting Aboriginal communities.
• Experience delivering or coordinating culturally appropriate programs and support.
• Experience in mental health, AOD, community services, care coordination or related support services.
• An ability to identify client needs and make appropriate referrals to health, AOD, mental health and community services.
• Strong communication and relationship-building skills with the ability to work effectively with clients, families and a range of stakeholders.
• Strong organisation and time-management skills, including the ability to manage competing priorities.
• Confidence maintaining client records and using computer-based data collection systems.
• A flexible, team-focused approach and willingness to undertake ongoing professional development.
• Current NSW Driver Licence
• Current Working with Children Check
• National Police Check
• Confirmation of Aboriginality
