As we continue to grow and expand our Assistive Technology and Home Modifications (ATHM) service, we’re looking for someone who is passionate about helping older Australians live
safely, independently and confidently at home. That’s where you come in.
About the Role
We’re looking for an Assistive Technology & Home Modifications (ATHM) Service Coordinator to join
our Care & Quality team.
This is a unique opportunity to play a central role in coordinating assistive technology and home modification solutions that help older Australians maintain their independence, improve
their quality of life, and continue living safely in the homes they love.
You’ll work closely with Members, their families and representatives, allied health professionals, suppliers, installers, builders and internal care teams to coordinate the end-to-end delivery of AT-HM services. From referral and funding through to procurement, installation and review, you’ll help guide Members through what can often be a complex journey with confidence and care. We’re looking for someone who combines strong coordination skills with a genuine passion for helping people.

Success in this role will be delivered in a variety of ways, including:

- Timely and effective coordination of referrals, approvals, installations and reviews.

What You’ll Be Doing
- Coordinating assistive technology and home modification referrals from intake through to completion.
- Supporting Members and their representatives to understand the ATHM process, funding options, expected timeframes and next steps.
- Working closely with allied health professionals, assessors, suppliers, installers and builders to coordinate assessments, quotations, procurement and installation activities.
- Monitoring progress and proactively managing service milestones to ensure a smooth Member experience.
- Maintaining accurate documentation and records across internal systems and relevant portals, including My Aged Care.
- Monitoring approved funding, quotations and costs to support appropriate budget utilisation and transparency.
- Identifying, documenting and escalating risks relating to installations, access, environments, safety and service delivery.
- Supporting reviews of ATHM solutions when Member needs change or additional support is required.
- Building strong relationships with Members, representatives, suppliers and internal teams.
- Contributing to quality improvement initiatives, audits and the ongoing development of our ATHM service.
- Representing Five Good Friends with pride, passion and professionalism.
About You
Experience:
- Using technology, e.g. computer-based system solutions, app-based programs and Artificial Intelligence (AI) tools, in the everyday work environment.
- Experience coordinating assistive technology and home modifications services, including intake, assessment support, implementation, review, and follow-up for older people or people with disability.
- Working knowledge of Support at Home, the Assistive Technology and Home Modifications (AT-HM) scheme, My Aged Care processes, and relevant aged care, disability, or community services pathways.
- Experience using digital platforms, client management systems, portals, scheduling tools, and documentation systems to track ATHM requests, approvals, budgets, suppliers, and outcomes.
- Experience working collaboratively with allied health/multi-disciplinary teams, assessors, builders, suppliers, installers, care teams, members, representatives, and external stakeholders.
- A proactive, solutions-focused approach and strong attention to detail.
Highly Desirable:
- Experience assessing, planning and coordinating care and support services for the aged or individuals with disability.
- Experience working in the Community Aged Care Sector, DVA or NDIS
- Qualifications in Allied Health, Nursing, Disability, Aged Care or a related human services field.
