Firstchance is seeking a passionate and experienced Coordinator of Supports to join our collaborative team.

This is an opportunity to work alongside families of children and young people with disability and developmental delay, helping them navigate the NDIS, connect with services, and build the confidence and skills to achieve their goals.

Part-time position available (3 days per week).

About the Role

As a Coordinator of Supports, you will work closely with families to help them understand and implement their NDIS plans, connect with appropriate supports, and build their capacity to navigate services with confidence. You will be part of a highly experienced and supportive team that values collaboration, professional growth and family-centred practice.

What You’ll Be Doing

  • Supporting participants and families to understand, implement and maximise their NDIS plans
  • Connecting families with mainstream, community, informal and funded supports
  • Building the confidence and capacity of families to make informed decisions and navigate service systems
  • Developing strong relationships with families, community organisations and service providers
  • Coordinating services and facilitating collaborative planning around participant goals
  • Assisting families during key transitions and periods of change
  • Preparing reports and documentation to support plan reviews and ongoing outcomes
  • Working within a strengths-based, family-centred framework that recognises each child’s unique strengths and potential

What We’re Looking For

You will bring:

  • Qualifications and/or demonstrated experience in NDIS, disability, community services or a related field
  • Knowledge of the NDIS and community support systems
  • Strong communication, relationship-building and advocacy skills
  • A commitment to family-centred, strengths-based practice
  • The ability to work both independently and collaboratively within a team environment

  • Current First Aid Certificate (or willingness to obtain)

Location

Based at our Mayfield office, with travel across Newcastle, Lake Macquarie and Port Stephens to support children, young people and families in their natural environments and local communities.
