We are seeking a passionate Support Coordinator to join our Spinal & Specialised Disability Support team in a 12-month fixed term role. In this role, you will work closely with participants, families, service providers and multidisciplinary teams to coordinate supports, overcome barriers and empower people to achieve their goals and live independently.
About the role
As a Support Coordinator at Estara, you will:
- Support participants to implement their NDIS plans and effectively access the supports and services they require
- Coordinate complex multidisciplinary supports to assist participants in achieving their goals
- Provide advice and consultation to participants, families and stakeholders in response to complex and challenging situations
- Support participants to address barriers, build capacity and increase meaningful community connections
- Develop and maintain strong working relationships with participants, families, carers, service providers and key stakeholders
- Liaise and collaborate with internal and external stakeholders to ensure coordinated and effective service delivery
- Contribute to a culture of client safeguarding, quality service delivery and continuous improvement
This role offers the opportunity to work with participants living with spinal cord injury, neurological conditions and other complex support needs, while being part of a highly experienced and supportive multidisciplinary team.
About you
You will bring:
- Tertiary qualifications in Human Services, Community Services, Social Work or a related discipline, or equivalent industry experience
- Demonstrated experience within the disability sector
- Strong understanding of person-centred practice and participant choice and control
- Highly developed interpersonal and communication skills, with the ability to build positive relationships with a diverse range of stakeholders
- Experience working effectively in an autonomous role
- Strong problem-solving, organisational and time management skills
- The ability to remain calm, professional and empathetic when supporting people through complex situations
- Knowledge of the NDIS Quality and Safeguards Framework will be highly regarded
- Current NDIS Worker Screening Check and Working with Children Check, or willingness to obtain prior to commencement.
