We are seeking a Post Placement Support Mentor to join our Transition to Work (TtW) team. This role is ideal for someone who thrives on proactive engagement and problem-solving, supporting young people after they commence employment or education to ensure their placement is successful and sustainable.
You will work closely with Participants, Employers, and Registered Training Organisations (RTOs), identifying risks early and delivering tailored interventions that keep young people engaged, confident, and progressing.
This is a hands-on, relationship-driven role for someone who makes a real impact through connection, advocacy, and action across our Mirrabooka & Morley sites.
Our ideal candidate will have:
- Demonstrated experience in employment services or a similar case management role, supporting disadvantaged Participants with multiple barriers to sustain work or education
- Strong interpersonal, communication and negotiation skills
- Confidence engaging with employers and training providers to support sustainable outcomes
- A proactive, solutions-focused approach to managing risk and disengagement
- The ability to work autonomously while contributing to a collaborative, outcomes-driven team
- Strong organisational and digital skills to maintain accurate documentation, records and ensure compliance with program guidelines and organisational procedures
- An understanding of the barriers young people face in maintaining work or education
- Qualifications in Youth Work, Community Development, Behavioural or Social Sciences are advantageous but not essential.
- Experience in Employment Services or Transition to Work is highly desirable.
