Disability Connect Victoria (DCV) is on the lookout for a passionate and organised Team Leader to help drive quality support. This is the perfect next step for a Support Worker ready to take on more responsibility while staying hands-on with client care.

You’ll still be on the roster, providing direct support, but you’ll also take the lead on key admin, communication, and coordination tasks that keep the program running smoothly. Think of yourself as the vital link between clients, support workers, and the office team.

This is a permanent part-time position based in Moe.

Shifts

This roster consists of up to 36 hours per week, commencing October 5th.

Day Shifts

Monday – 7:00am to 3:00pm

Tuesday – 7:00am to 3:00pm

Wednesday 10:00am to 2:00pm

Thursday 7:00am to 3:00pm

Friday 7:00am to 3:00pm

About the role:

Provide Assistance with community access

Provide assistance with meal preparation

Domestic assistance

Provide support with daily living

Medication assistance

Supporting the participant in line with their individual support plan

Communicating effectively with the participant, their family and support team

What you’ll bring:

•Certificate IV in Disability is preferred

* Experience with hoist slings

* Experience in Diabetes management

* Experience in Asthma management

• A strong commitment to supporting people with disability

* Strong initiative

• Excellent communication, organisation, and admin skills

• Confidence working with systems, scheduling, and documentation

• A calm, flexible approach to managing the unexpected

• A team mindset
