We foster an inclusive, community-centred environment where diversity is celebrated, individuality is respected, and every person is supported to live with dignity, choice, and independence. Through compassionate, person-centred practice, we strive to create meaningful opportunities for connection, equality, and belonging.

We are looking for an experienced Registered Nurse Team Leader who is ready to bring clinical expertise, leadership, and a commitment to quality care to a dedicated nursing team supporting a participant with complex clinical and disability-related needs.

If you are passionate about developing others, strengthening clinical practice, and helping shape a consistent, high-quality care environment, this is an opportunity to put your leadership into meaningful action.

About the Opportunity

This is more than a clinical nursing position. It is an opportunity to help establish, guide, and strengthen a dedicated nursing team in a Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA) environment in Jordan Springs, Penrith, NSW.

Working closely with the nursing team and relevant stakeholders, you will help coordinate clinical care, promote safe and consistent practice, support staff development, and ensure individualised care plans are implemented effectively.

You will play an important role in maintaining clinical standards, supporting sound decision-making, identifying emerging risks, and promoting continuity of care for a participant with complex health and disability-related needs.

The service operates within a 24/7 SIL/SDA support environment, with nursing coverage requirements that may include sleepover arrangements. Your specific roster, leadership responsibilities, and coordination arrangements will be discussed during recruitment.

This opportunity is ideal for a clinically experienced nurse who enjoys combining hands-on nursing practice with team leadership, mentoring, and service coordination.

  • Monitor clinical documentation and care delivery, identifying opportunities to strengthen safety and quality.
  • Support the timely recognition, documentation, and escalation of clinical deterioration and emerging health concerns.
  • Encourage evidence-informed practice, clinical accountability, and continuous improvement.

Complex Care Planning and Clinical Coordination

  • Coordinate and support the implementation and review of individualised nursing care plans in collaboration with relevant stakeholders.
  • Guide consistent approaches to diabetes management, blood glucose monitoring, insulin administration, wound care, catheter and continence care, pressure care, and lymphoedema management.
  • Promote safe medication administration, clinical monitoring, infection prevention, and accurate recordkeeping.
  • Ensure nurses understand the participant’s clinical requirements, individual preferences, escalation pathways, and approved procedures.
  • Support safe bariatric care and complex manual handling practices, including hoist and sling transfers, in accordance with approved procedures.

Team Mentoring and Professional Development

  • Mentor and guide Registered Nurses in clinical decision-making and person-centred practice.
  • Support staff induction, knowledge sharing, and consistent application of clinical procedures.
  • Identify training and development needs and encourage ongoing professional growth.
  • Foster a collaborative environment built on accountability, respectful communication, and shared responsibility.
  • Promote reflective practice and a culture of continuous learning.

Nursing Roster and Service Coordination

  • Contribute to nursing roster coordination to support continuity of care and appropriate clinical coverage.
  • Identify potential coverage gaps and communicate roster requirements to relevant operational staff.
  • Facilitate effective handovers and communication across the nursing team.
  • Help maintain consistent clinical support across the agreed roster, including relevant overnight and sleepover arrangements.
  • Work collaboratively with operational leaders to support reliable and well-coordinated service delivery.

Stakeholder Engagement and Clinical Governance

  • Communicate professionally with relevant health professionals, support teams, and other stakeholders.
  • Support timely and accurate clinical records, incident reports, and escalation documentation.
  • Contribute to quality assurance, clinical risk management, and continuous improvement initiatives.
