Bidgerdii Community Health Services is an Aboriginal and Torres Strait Islander community controlled primary health care service, operating in the Rockhampton and wider Central Queensland region for more than 30 years and our vision is for Healthy and Strong Aboriginal and Torres Strait Islander, children, families and communities – self – determining, culturally strong, and thriving in their own way. The name ‘Bidgerdii’ means ‘healing plant’ – the bark of the plant was used for tobacco, and the leaves for healing sores and rashes.
Bidgerdii provides comprehensive primary healthcare and wellbeing services, collaborates with partner agencies for integrated continuity of care and advocates for health policy and health system reform. Our services include general practice bulk-billing clinics, social and emotional wellbeing and generalist counselling services, chronic disease management, preventative health screening, children and family health and early childhood education services.
About the Role:
This position is located in Emerald and forms part of the Integrated Team Care / Allied Health Service work unit team which is responsible for working collaboratively with clients, general practitioners, general practice/Community-Controlled Health Service staff and a broad range of other relevant government/non-government services, to facilitate provision of appropriate multidisciplinary care for Aboriginal and/or Torres Strait Islander people with target chronic diseases, to help them manage their health in a way that meets their individual needs and will result in improved health outcomes.
Care Coordinators are expected to identify when a client’s condition may require further assistance from a health professional. The role of the care coordinator should develop and maintain a close relationship with their clients GP and work with clients to improve their capability to engage with the broader health system.
The aim of care coordination is to provide a level of non-clinical care and assistance that would otherwise not be available to clients with complex chronic care needs.
Essential Criteria
- Certificate IV or higher qualification in Health, Community Services, Aboriginal Health, Nursing, Allied Health, or a related discipline. ** Or relevant work experience in the sector **
- Current AHPRA Registration (or eligibility for registration where applicable). Immunisation Endorsement desirable where a Diploma of Nursing is held.
- Demonstrated understanding of chronic disease management and care coordination within a primary health care setting.
- Knowledge and appreciation of the current health reform environment and commitment to improving health outcomes for Aboriginal and Torres Strait Islander peoples and communities.
- Strong organisational and time management skills, with the ability to coordinate care across multiple services and providers.
- Demonstrated ability to advocate for clients and support improved access to culturally safe and appropriate health services for people of all ages.
- Excellent communication and interpersonal skills, with the ability to build strong, respectful relationships with Aboriginal and Torres Strait Islander clients, families, communities, and stakeholders.
- Ability to provide culturally responsive and trauma-informed support to clients living with chronic and complex health conditions.
- Proven ability to work collaboratively within a multidisciplinary team and engage effectively with internal and external health professionals, community organisations, and specialist services.
- Sound understanding of general practice, Aboriginal Community Controlled Health Services (ACCHS), and the broader primary health care environment.
- Ability to maintain accurate client records, contribute to care planning, and support continuity of care initiatives.
Desirable Criteria
- Previous experience in a Chronic Disease Care Coordinator, Aboriginal Health Worker, Community Services, Nursing, Allied Health, or similar health-related role.
- Experience working within an Aboriginal Medical Service or with Aboriginal and Torres Strait Islander clients and communities.
- Understanding of Closing the Gap initiatives, preventative health programs, and chronic disease frameworks relevant to Aboriginal and Torres Strait Islander health.
- Experience supporting clients with chronic and complex conditions, including care planning, referrals, follow-up, and health education.
Practical Requirements:
- Valid Blue Card (Working with children check)
- Current C Class Driver’s Licence (QLD) – essential
- Immunisation requirements – It is a condition of employment for this role for the employee to be, and remain, vaccinated against the following vaccine-preventable diseases: Measles, Mumps, Rubella, Varicella, Hepatitis B, Hepatitis A, and pertussis (whooping cough) during their employment.
- Satisfactory Police Criminal History Check.
