Full-Time | 2-Year Contract | Ashfield, NSW Potential for Permanent Appointment

Ashfield Baptist Homes (ABH) is seeking an experienced and values-driven Community Care Manager to lead our growing Home and Community Care operations.

This is a senior operational leadership opportunity for an experienced aged care or community care professional who can combine people leadership, operational performance, clinical governance, quality and compliance to deliver exceptional outcomes for clients.

The position is initially offered as a two-year full-time contract, with the potential for the role to transition to a permanent position.

About The Role

As Community Care Manager, you will provide strategic and operational leadership across a range of community services, including:

  • Support at Home
  • Commonwealth Home Support Programme (CHSP)
  • Aged Care Volunteer Visitors Scheme (ACVVS)

You will lead a multidisciplinary team comprising Care Partners, Care Coordinators, Coordination staff, Support Workers and designated clinical staff, ensuring services are safe, responsive, person-centred and operationally sustainable.

Reporting to Executive Management, you will play a key role in strengthening service delivery, workforce capability, quality, governance and organisational performance.

Key Responsibilities:

Operational Leadership

  • Lead the day-to-day operations of Home and Community Care services
  • Provide clear direction, leadership and support to Care Coordinators, Coordinators, Support Workers and designated clinical staff
  • Drive performance across client experience, workforce effectiveness, quality, safety and operational outcomes
  • Monitor service activity, client hours, workforce utilisation and operational performance
  • Identify operational risks, trends and opportunities for improvement
  • Ensure services are responsive, reliable, efficient and financially sustainable

Client Experience & Service Delivery

  • Ensure client needs, goals, preferences and care plans are translated into coordinated and effective service delivery
  • Promote a strong person-centred culture focused on dignity, choice, independence and quality of life
  • Monitor client experience and respond effectively to feedback, complaints and emerging concerns
  • Ensure services are delivered consistently and in accordance with client expectations and organisational standards.

Quality, Governance & Compliance

  • Lead quality, governance, incident, complaint and continuous improvement activities across community programs
  • Ensure compliance with applicable legislation, funding requirements, professional standards, the Aged Care Quality Standards, Aged Care Act and organisational policies and procedures
  • Lead the clinical governance framework for Home Care
  • Ensure clinical services are delivered within appropriate professional scope of practice
  • Monitor compliance, identify gaps and implement corrective and preventative actions
  • Maintain a strong culture of accountability, risk management and continuous improvement.

People Leadership & Capability

  • Lead, develop and support a diverse community care workforce
  • Establish clear performance expectations and accountability
  • Oversee staff competency assessment, training, supervision and professional development
  • Build a positive, collaborative and high-performing team culture
  • Identify workforce capability requirements and support sustainable workforce planning
  • Lead teams through change while maintaining service quality and staff engagement.

Reporting & Performance

  • Provide clear, accurate and timely operational and performance reporting to Executive Management
  • Analyse workforce, client, service delivery, financial and quality data to support informed decision-making
  • Translate operational data into practical improvement strategies
  • Contribute to strategic planning and the ongoing development of ABH’s community care services

About You

What You’ll Bring

Essential

  • Minimum 5 years’ experience in aged care, community care or a related health/community services environment
  • Demonstrated experience leading client services and operational teams
  • Strong understanding of aged care legislation, compliance and quality requirements
  • Demonstrated understanding of Support at Home and contemporary community care models
  • Experience in quality, governance, incident and complaint management
  • Demonstrated experience in workforce leadership, performance management and staff development
  • Strong written and verbal communication skills
  • Ability to analyse operational information, identify risks and drive improvement
  • Current Australian driver’s licence
  • Current National Police Check
  • Willingness to obtain a Working with Children Check, if required
  • Right to work in Australia.

Desirable

  • Relevant tertiary qualification in aged care, health, nursing, community services, management or a related discipline
  • Clinical qualification or experience in clinical governance
  • Experience working across multiple community care funding programs
  • Experience leading organisational change and continuous improvement initiatives.
