Amity Health are seeking a compassionate and organised Client Liaison Officer to support access to our care finder program across regional Western Australia.

In this role, you will act as a critical first point of contact for clients, carers, and referrers – ensuring people are welcomed and connected to the right support. You will help enable equitable access to aged care services, particularly for those experiencing disadvantage, isolation, or barriers to navigating the system.

About care finder

Funded by the Australian Government and delivered by Amity Health on behalf of WA Primary Health Alliance, the care finder program provides specialist, face-to-face support to vulnerable older people who cannot navigate the aged care system independently.

care finders work alongside clients to understand their needs, explain aged care options, complete My Aged Care processes, and connect them with appropriate services. The program focuses on people experiencing complex challenges, disadvantage, or social isolation, ensuring they receive personalised, intensive, and trusted support.

About the Role

This is a meaningful role where you will play a key part in helping clients access the right support. As a care finder Client Liaison Officer, you will:

  • Provide a warm, trauma-informed and person-centred first response to clients, carers and referrers
  • Conduct eligibility screening and ensure alignment to care finder program criteria
  • Gather key information including presenting needs, risks, and communication preferences
  • Identify and escalate immediate risks or concerns appropriately
  • Support individuals who are not eligible by providing alternative pathways and referrals, ensuring no one is left without support
  • Support clients and referrers to understand the intake process, wait times, and available supports
  • Provide basic guidance on My Aged Care and aged care pathways within scope
  • Facilitate the discharge process and follow-up check-ins, ensuring continuity of care

This is a part-time, 4 day per week role located in Albany, WA.

About You

You are a compassionate and detail-oriented professional who thrives in a client-facing, intake and coordination role. You bring:

  • Experience in community services, aged care, mental health, or health settings
  • Strong understanding of vulnerability, disadvantage, and barriers to care
  • Proven ability to manage intake processes, triage, and service navigation
  • Excellent communication skills, with the ability to provide clear, supportive guidance
  • Strong organisational skills and the ability to manage workflow independently
  • A person-centred, trauma-informed and strengths-based approach
  • Confidence using client systems and maintaining accurate records

Essential Requirements

  • Tertiary qualification in Social Work, Psychology, Community Services or related field
  • Current National Police Clearance
  • Current WA Driver’s Licence
