Dementia Care Clinical Nurse Specialist

The Dementia Foundation is seeking a compassionate, experienced, and highly motivated Dementia Care Clinical Nurse Specialist to join our growing team. This rewarding role provides clinical leadership and nursing support across our Supported Independent Living (SIL) homes and community-based care programs, helping individuals living with dementia maintain the highest possible quality of life, independence, dignity, and wellbeing.

As a trusted clinical professional, you will work closely with participants, families, support staff, allied health professionals, and external healthcare providers to deliver person-centred care that aligns with best practice dementia support. You will regularly visit SIL homes and community clients, provide clinical oversight, conduct assessments, develop care plans, support staff capability development, and ensure compliance with NDIS, aged care, and healthcare standards.

What You’ll Be Doing

• Conducting regular clinical visits to Supported Independent Living homes and community-based clients
• Undertaking comprehensive nursing assessments and developing individualised care plans
• Monitoring participant health, wellbeing, risks, and changing care needs
• Providing clinical guidance, coaching, and education to support staff and care teams
• Supporting medication management, health coordination, and clinical documentation
• Liaising with families, General Practitioners, specialists, hospitals, and allied health professionals
• Supporting quality improvement initiatives and ensuring compliance with NDIS Practice Standards and organisational policies
• Promoting evidence-based dementia care practices and positive behaviour support approaches
• Contributing to service development and continuous improvement across The Dementia Foundation’s programs

What You’ll Bring

We’re looking for a dedicated nursing professional who is passionate about delivering exceptional dementia care and building strong relationships with participants, families, and multidisciplinary teams.

Key requirements include:

• Current registration as a Registered Nurse with AHPRA
• Demonstrated clinical experience supporting people living with dementia, cognitive impairment, or complex support needs
• Strong assessment, care planning, and clinical decision-making skills
• Sound knowledge of person-centred care principles and contemporary dementia care practices
• Excellent communication and relationship-building abilities
• Strong organisational and time management skills with the ability to work autonomously across multiple locations
• Experience working within the disability, community care, aged care, or NDIS sectors
• High levels of professionalism, empathy, discretion, and clinical judgment
• Ability to maintain accurate clinical records and documentation
• Tertiary nursing qualifications and ongoing commitment to professional development

Essential Requirements:

• Current AHPRA Registered Nurse registration
• Full Australian working rights
• Current NDIS Worker Screening Check
• Current Working with Children Check (where applicable)
• Current Australian Driver’s Licence
• Current First Aid and CPR certification

Desirable:

• Qualifications or additional training in dementia care, gerontology, mental health, or disability support
• Experience working within Supported Independent Living (SIL) services
• Knowledge of NDIS Practice Standards, Quality Indicators, and Restrictive Practices requirements
