About Us

360 Health Management is a regional disability support provider servicing Swan Hill, Bendigo, Shepparton and Seymour, offering a range of services to our NDIS participants from Support Coordination, Behaviour Support, Respite and much more.

The Role:

You will be responsible for managing and coordinating all supports for your caseload of participants, in line with their NDIS plan, individual goals and challenges. You will develop strong rapport with clients and their support networks to ensure you fully understand them and how best we can support them to understand and navigate the complexity of the NDIS and their personal plan.

Key Responsibilities:

  • Assist participants in navigating social care services and resources.
  • Coordinate all aspects of supports, from service agreements to schedules and budgets
  • Build strong relationships with providers, therapists, and health professionals to ensure quality care
  • Provide administrative support to the social care team, including scheduling appointments and maintaining records.
  • Conduct initial assessments to identify client needs and develop appropriate support plans.
  • Collaborate with external agencies and service providers to ensure comprehensive care for clients.
  • Stay up to date with local services and resources to connect participants with the right support.
  • Maintain accurate documentation and case notes in compliance with organisational policies.
  • Participate in team meetings and training sessions to enhance service delivery and personal development.

About you:

  • Agile – no two days are ever the same
  • Work well under pressure, as you are working with someone’s life, things will often move and change, requiring ability to navigate this to support clients
  • Strong attention to detail and love for numbers
  • Outstanding organisation and time management skills, to ensure that all participants are being supported equally

Role Requirements:

  • Relevant certificate (of higher) within Social, Community or Disability support
  • Proven relatable experience working within the NDIS field (ideal)
  • Proven experience in a consumer-focused role meeting the needs of complex clients
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Unrestricted Australian driver’s license and access to a reliable vehicle
