About Us

Here at 360 Health Management, we pride ourselves in being a strong regional disability support provider servicing Shepparton, Bendigo, Swan Hill and Seymour. We provide a range of services to our NDIS participants from Community Access and in Home Support, to Support Coordination, Behaviour Support, Allied Health and much more.

We are looking for dedicated and reliable Support Workers with previous experience who are eager to work with participants of all ages and needs. With shifts covering all hours of the day, including sleepovers in a SIL house.

Responsibilities:

  • Assist clients with daily living activities, including personal care, meal preparation, and household tasks.
  • Support clients in accessing community resources and participating in social activities.
  • Complete daily shift notes to accurately detail client progress and steps towards their goals
  • Report any changes in behaviour or health to the supervising staff.
  • Collaborate with other healthcare professionals to ensure comprehensive care for clients.
  • Provide emotional support and encouragement to clients, fostering a positive and inclusive environment.

Qualifications:

  • A relevant qualification in disability support, social care, or a related field is preferred.
  • Previous experience in a similar role.
  • Strong communication and interpersonal skills to effectively engage with clients and their families.
  • Ability to work independently and as part of a team, demonstrating flexibility and adaptability.
  • A valid Australian driver’s licence and access to a vehicle may be required for community support roles.
