Join our Pacific Navigation Service and support Whanau to access primary health care, strengthen wellbeing, and achieve better long-term health outcomes.

About the role

We’re looking for a Registered Nurse who is passionate about improving health outcomes for Pacific peoples. In this community-based role, you’ll empower individuals and whanau to navigate the health system, access primary care, and build confidence in managing their health and wellbeing.

Working alongside a dedicated multidisciplinary team—including social workers, community health workers, and Pacific health partners—you will:

  • Provide clinical assessment, early intervention, and health coaching
  • Support individuals and families with chronic condition management
  • Facilitate access to primary and community health services
  • Deliver culturally responsive health education
  • Promote lifestyle change and self-management
  • Build strong relationships with general practice teams and Pacific communities

This role is based in Porirua/Wellington and contributes to strengthening equity and improving health outcomes across the CCDHB region.

About you

You will thrive in this role if you are:

  • A New Zealand Registered Nurse with a current APC
  • Experienced in working with Pacific communities and understanding the Pacific worldviews of health
  • Skilled at building trusting relationships with whanau, communities, and stakeholders
  • Able to work both independently and collaboratively in a multidisciplinary environment
  • Culturally responsive, adaptable, and committed to equity
  • Knowledgeable in Pacific health models (e.g. Fonofale, Fa’afaletui)
  • Confident with digital health systems and clinical, documentation
  • A clean full driver’s licence is required
